Release Forms

Any time we go on an activity, there is an inherent risk. While we have a tremendous adult team that takes an extreme amount of care to insure the safety of everyone involved, accidents do happen. Because accidents happen, we have two forms we require everyone to fill out in order to participate in activities with us. One is a medical release form which will be used in the event we need to seek medical treatment for your teen, and the second is what we call a consent form which lets us know that parental permission has been granted to join us on activities.
